    2. Detailed Description: [Include a comprehensive description of the physical appearance, clothing, and any distinguishing features of the child].

    5. Immediate Actions Taken: [Describe the immediate measures taken to ensure the child’s safety, such as contacting emergency services or child protection agencies].

    7. Notification of Authorities: [Specify the authorities contacted immediately after the discovery, such as the police, child welfare services, or other mandated departments].

    8. Interview with Witnesses: [Document any interviews conducted with witnesses regarding the circumstances surrounding the child’s abandonment].

    9. Efforts to Identify Parents: [Outline any investigations or inquiries made to identify the child’s birth parents, including searches in relevant databases or interviews with potential relatives or acquaintances].

    10. Placement and Custody: [Indicate the interim placement of the child, whether it is within a child protection facility, foster care, or a suitable alternative arrangement].

    11. Legal Proceedings: [Detail any legal processes initiated to determine the child’s status, which may include a court petition for guardianship, DNA testing, or other necessary legal actions].

    12. Documentation of Abandonment: [Summarize the evidence gathered to substantiate the child’s abandonment, such as witness statements, photographic evidence, or any other supporting documentation].

    These minutes have been prepared in accordance with established protocols and procedures to ensure accurate recording of data and compliance with legal requirements. They shall serve as a vital record for future reference and may contribute to the child’s well-being and eventual establishment of a birth certificate, as deemed necessary.

Applicants may collect their results at the Commune People’s Committee office or opt for a delivery option by registering with the Commune People’s Committee and paying the applicable service fee.

Settlement Agency Loses Funding

According to Article 5 of the Joint Circular No. 05/2015/TTLT-BTP-BCA-BYT 2015, the One-Stop-Shop Section of the Commune People’s Committee is responsible for receiving the necessary documents, such as the health insurance card for children under 6 years old. This unit will handle the reception, processing, and transfer of the dossiers to the district social insurance agency in order to issue the health insurance cards for children under 6 years old.
